La jugabildad modifica las estadísticas del personaje del jugador, proporcionándole o modificando habilidades, entre otras cosas. Los objetos en general están clasificados en un numero distinto de tipos y grupos. Algunos objetos son pasivos, que añaden un efecto permanente a Isaac, mientras otros solo se activan
Los objetos que estén tirados en The Binding of Isaac NUNCA desaparecen con el tiempo; una moneda tirada se quedara ahí hasta que la recojas o salgas del nivel. Un objeto tirado puede dejarse en su lugar para recogerlo después.
Entre esos tipos están:
Objetos Activables
Los objetos activables aparecen en el cuadro en donde dice: "Space". Solo se puede tener uno a la vez. Un objeto activable se usa al presionar la barra espaciadora y, a diferencia de una píldora o carta, estos objetos se pueden recargar y volverse a usar.(Aunque hay algunos que son de un solo uso y luego de este desaparece). Estos objetos tienen una barra dividida que muestran cuanto falta para volver a utilizar el objeto, una vez que se despeje un cuarto lleno de enemigos, esta recargara una parte, o también puedes agarrar una pila y recargar el objeto instantáneamente. Algunos objetos tienen la barra completa (osea, cuando se use se podrá volver a utilizar una vez despejada una habitación) y otros objetos divididos hasta en 6 partes para recargarse. Muchos de los objetos activables solo aparecen una vez por partida con algunas excepciones, así que dejar un objeto activable atrás es tipicamente una elección permanente.

Píldoras
Estas se encuentran debajo a la izquierda en la pantalla, se activan con Q y son objetos de un solo uso. (Aunque se pueden usar muchas veces con el item recientemente añadido ''Placebo'') Hay una cantidad de píldoras de varios estilos y efectos. Al principio del juego cada píldora se le asigna un efecto aleatorio, el cual permanece en esa píldora hasta que se acabe el juego. Las píldoras aparecen no identificadas al recogerlas, a menos que el jugador tenga el objeto ''PHD'', que identifica la píldora después de un pequeño retraso. Si ya has recogido un tipo de píldora y la has usado, al volver a recogerla saldrá el nombre. Recoger una píldora o tarjeta mientras se tiene una hará que se deje la píldora vieja en el suelo.
Cartas de tarot
Las cartas de Tarot también van en el mismo lugar y se activan al igual que las pastillas, e igual son de un solo uso. (Al igual que las pildoras con el item ''Blank Card'' Las cartas se puden usar varias veces). Las cartas de Tarot no se pueden identificar hasta que se recogen. Cada carta tiene un efecto único y predeterminado. Recoger una carta o píldora mientras se tiene otra hará que se deje la carta vieja en el suelo. (Esto se puede cambiar con algun item)
Abalorios
Los Abalorios o Trinkets tienen su propio espacio, son de efecto pasivo y afectan a Isaac de formas únicas. Solo se puede llevar uno a la vez (A menos de que se tenga ''Mom's Bag'', que permite a Isaac llevar 2 abalorios).
Recolectables
Son objetos comunes que aparecen en el juego y al recogerse se usan automáticamente. En esta categoría están los corazones rojos, azules y negros, centavos, llaves y bombas. Desaparecen al recogerse.
Coleccionables
Hay 198 objetos coleccionables en el juego. Aquí hay una lista de todos los coleccionables, separados en categorías. Algunos coleccionables de pene pueden cambiar el aspecto de tu personaje al adquirirse. Los coleccionables que encuentres los puedes ver en la pagina de Coleccionables
